If I want to make a DVD and take photos for Sky Diving How much will it cost me?

Hi i'm looking for a package activity, which is like foursome combo.
I'm little bit confused with the height of sky dive, is it 14,000 ft or 11,000 ft for the package?
'description' says 14,000 while 'trip includes' shows 11,000 ft.

also, what if I want to do introdive during the reef tour, how much of extra cost would be required?
